Health Vendor Training: Special Incident Reports
Learn about CA Title 17 and SARC requirements for reporting incidents, types of incidents, how to access the form, and all about the process.
This is a 3-hour training in person at the SARC San Jose Office.
Certificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct.
Space is limited- 20 people maximum at this time.
2 people per facility to allow for more agencies to attend.

Date: Wednesday January 11th Time: 9:30 am to 12:30 pm
Please arrive no later than 9:15 for COVID symptom screening. Park in east lot (not San Ignacio) and enter through the “back” door.
Take an at-home COVID test before you come. Bring a photo of the results and bring your vaccination card (photo or ecard is ok). Wear a mask please.
