Vendor Training: Thinking Ahead About End-of-Life Issues
The DDS program, “Thinking Ahead” will be used as a basis for this class. Bioethics Team meetings and proper use of the POLST will also be discussed.
This is a 3-hour training in person at the SARC San Jose Office.
Certificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct.
Space is limited- 20 people maximum at this time.
2 people per facility to allow for more agencies to attend.

Please arrive early to have COVID symptom check and show your vaccination card and negative test results. Please do not attend if you have any COVID symptoms!
