DESCRIPTION:Independent Living Services New Vendor Orientation (ILNV)  \nJanuary 10\, 2023 from 9:00 a.m. to 3:30 p.m.  \n  \nPLEASE CAREFULLY READ THIS POST IN ITS ENTIRETY. FAILURE TO FOLLOW ALL INSTRUCTIONS ON THIS POST WILL RESULT IN AUTOMATIC DENIAL OF ADMISSION TO THE ILNV.  \n  \nRegistration is now open for the January 10\, 2023 Independent Living Services New Vendor Orientation (ILNV). The ILNV will be conducted in person at SARC’s San Jose office. Successful completion of the ILNV is required for vendorization as an Independent Living Services provider. \n  \nTo register for the ILNV\, please complete each of the following items by no later than 11:59 p.m. on Friday\, December 16\, 2022: \n  \n\nRegister for the ILNV using the link found at the bottom of this post.\nSend an email to Gabriela Alvarez at galvarez@sarc.org that contains the following attachments in support of your request for admission to the ILNV:\n\nA Letter of Intent that explains:\n(1) why you are interested in providing Independent Living Services to SARC-sponsored individuals\,\n(2) your background and experience working with DD individuals\, and\n(3) how you will implement the HCBS Final Rule in providing services to SARC-sponsored individuals.\nYour updated resume.\n\n\n\nEach of the described documents must be sent as a separate Word or pdf attachment to your email. Letters of Intent or resumes included as in-line text in your email will not be accepted. \nPlease make sure that your Letter of Intent and resume are free of typographical errors before submission. You must register for the ILNV through the link below AND submit all supporting documents described above by no later than 11:59 p.m. on December 16\, 2022 in order to be considered for admission. Registrations and supporting documents timestamped after 11:59 p.m. on December 16\, 2022 will not be considered for admission.  \nYou will receive an email by end of day on December 27\, 2022 that either confirms or denies your admission to the January ILNV. If you are admitted\, you will receive instructions on where to park\, where to enter the building\, our COVID protocol\, and the training agenda. \nPlease note that no more than two (2) individuals from a single agency will be admitted to the ILNV unless prior approval is obtained from Community Resources Unit Manager\, Ann Sieber. \nWe do not maintain a waitlist for admission. If you are not admitted to or are unable to attend the January 2023 ILNV\, you will need to re-apply for admission to the next ILNV scheduled for February 14\, 2023. Registration for the February ILNV will open in January. Check the Events Calendar on SARC’s website for further information about future ILNVs.

DESCRIPTION:Health Vendor Training: Special Incident Reports \nLearn about CA Title 17 and SARC requirements for reporting incidents\, types of incidents\, how to access the form\, and all about the process. \nThis is a 3-hour training in person at the SARC San Jose Office. \nCertificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct. \nSpace is limited- 20 people maximum at this time. \n2 people per facility to allow for more agencies to attend. \n*Register each person separately \nDate: Wednesday January 11th Time: 9:30 am to 12:30 pm \nPlease arrive no later than 9:15 for COVID symptom screening. Park in east lot (not San Ignacio) and enter through the “back” door. \n Take an at-home COVID test before you come. Bring a photo of the results and bring your vaccination card (photo or ecard is ok).
