DESCRIPTION:Special Incident Reports \nLearn the basics about Title 17 requirements as well as SARC standards of practice for Special Incident Reporting. \nThis is a 3-hour training in person at the SARC San Jose Office. \nCertificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct. \n Date: Friday June 24 \nTime: 9:30 am to 12:30 pm \nPlease arrive early to have COVID symptom check and show your vaccination card.   \nMASK IS REQUIRED AT ALL TIMES \nPlease do not attend if you have any COVID symptoms! \nLocation:  6203 San Ignacio Avenue\, San Jose 95119 \nPlease wait outside at the back door. \nBring a pen/pencil and a jacket or sweater. There will be a 10 minute break. \nONLY 2 PEOPLE FROM EACH FACILITY CAN REGISTER FOR A CLASS \nThis is to allow for people from other homes and agencies to attend.  Each person registers separately and must have an email address.  \n20 people maximum.  \nPlease Click here to Register
