The dates for the class are May 1st\, 2nd\, and 3rd\, 2023. You must attend all three classes. \nThe Residential Services Orientation will take place in person at the San Andreas Regional Center San Jose office. \nTo be registered you must complete the following items: \n\n\n\nRegister (registration link can be found at the bottom of the post)\nSend a letter of intent to Ruby Hernandez at rhernandez@sarc.org and Gabriela Alvarez at galvarez@sarc.org in a separate word document (letters of intent sent as emails will not be accepted).\nSend a resume to Ruby Hernandez at rhernandez@sarc.org and Gabriela Alvarez at galvarez@sarc.org in a separate word document (resumes sent as emails will not be accepted).\nSend a copy of your Community Care Licensing (CCL) administrator certificate to Ruby Hernandez at rhernandez@sarc.org and Gabriela Alvarez at galvarez@sarc.org\nSend a copy of your Direct Support Professional (DSP) training year 1 and 2 certificates to Ruby Hernandez at rhernandez@sarc.org and Gabriela Alvarez at galvarez@sarc.org\n\nThe letter of intent must include: (if any question is not answered\, it will result in automatic denial)\n\n\n\nWhy do you want to become a residential provider?\nWhat is your background and experience that prepared you for this position?\nWhat steps have you taken with CCL (Community Care Licensing)?\nPlease describe the level of service you wish to provide.\nHow will HCBS Final Rule impact the way you are going to provide services?\n\n\n\n\nPlease conduct a grammar and spell check to your letter of intent. We will not accept any letters of intent or registrations after 11:59pm\, Friday\, April 14\, 2023. \nThere are no waitlists. The next Residential Services Orientation will be on August 7th\, 8th\, and 9th\, 2023. Registration will be open in mid-June\, and you will find the registration post in the month of August on SARC’s events calendar. \n\n\nOnly two individuals per agency unless previously approved by the Community Services Unit Manager\, Ann Sieber. \n\n\nBy the end of day Friday\, April 21\, 2023\, you will be receiving a confirmation letter or denial letter via email confirming whether or not you are in the class. Along with the confirmation letters will be instructions on getting into the building/signing in\, and schedule of the presenters. \n*NOTE: If we have less than 5 registrants\, the May RSO will be canceled. Any registrants who were approved to attend will be automatically added to the list for the August RSO. You will not need to sign up again or resubmit any documents. \n*DO NOT ATTEND IF YOU ARE EXPERIENCING COVID SYMPTOMS

DESCRIPTION:Supported Living Services New Vendor Orientation (SLNV) \nClass Date & TIME: May 23\, 2023 from 9:30 a.m. to 3:30 p.m. \nPLEASE CAREFULLY READ THIS POST IN ITS ENTIRETY. FAILURE TO FOLLOW ALL INSTRUCTIONS IN THIS POST WILL RESULT IN AUTOMATIC DENIAL OF ADMISSION TO THE SLNV. \nRegistration is now open for the May 23\, 2023 Supported Living Services New Vendor Orientation (SLNV). The SLNV will be conducted in person at SARC’s San Jose office. Successful completion of the SLNV is one of the requirements for vendorization as a Supported Living Services provider. \nTo apply for admission to the SLNV\, please complete each of the following items by no later than 11:59 p.m. on Tuesday\, May 9\, 2023: \n1. RSVP for the SLNV using the link found at the bottom of this post. Please note that RSVPing does not guarantee admission to SLNV. \n2. Send an email to Gabriela Alvarez galvarez@sarc.org and Kai Reade kreade@sarc.org\, respectively\, that contains the following attachments in support of your request for admission to the SLNV: \n\nA Letter of Intent that explains:\n\n(1) why you are interested in providing Supported Living Services to SARC-sponsored individuals\,\n(2) your background and experience working with individuals with ID/DD\,\n(3) the county or counties within the SARC catchment area in which you intend to provide services\,\n(4) steps you have taken in preparation to provided services to SARC-sponsored individuals\,\n(5) how you will implement and support SARC-sponsored individuals’ preferences and choices as required by the HCBS Final Rule.\n\n\nb. Your updated resume.\n\nEach of the described documents must be sent as a separate Word or pdf attachment to your email. Letters of Intent or resumes included as in-line text in your email will not be accepted. Please make sure that your Letter of Intent and resume is free of typographical errors before submission. \nYou must register for the SLVN through the link below AND submit all supporting documents described above by no later than 11:59 p.m. on May 9\, 2023 in order to be considered for admission. Registrations and supporting documents timestamped after 11:59 p.m. on May 9\, 2023 will not be accepted or considered. \nYou will receive an email by end of business day on May 16\, 2023 that either confirms or denies your admission to SLVN. If you are admitted\, you will receive instructions on where to park\, where to enter the building\, our COVID protocol\, and the training agenda. Please note that no more than two (2) individuals from a single agency will be admitted to SLVN unless prior approval is obtained from Community Resources Unit Manager Ann Sieber. \nA minimum of three (3) people must be admitted to any scheduled SLVN to avoid cancellation. If you are admitted to a SLVN that is canceled due to low registration/admission\, you will be notified of the cancellation and placed on the attendance list for the next orientation. \nIf you are not admitted to the May 2023 SLVN\, or if you are admitted but fail to attend\, you will need to re-apply for admission to a future SLVN.
