DESCRIPTION:Vendor Training: Thinking Ahead About End-of-Life Issues \nThe DDS program\, “Thinking Ahead” will be used as a basis for this class. Bioethics Team meetings and proper use of the POLST will also be discussed. \nThis is a 3-hour training in person at the SARC San Jose Office. \nCertificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct. \nSpace is limited- 20 people maximum at this time. \n2 people per facility to allow for more agencies to attend.  \n*Register each person separately \n\nDate: Tuesday November 22nd Time: 9:30 am to 12:30 pm\n\nPlease arrive early to have COVID symptom check and show your vaccination card and negative test results.
