Vendor Training: <\/strong>Thinking Ahead About End-of-Life Issues<\/strong><\/p>\n

The DDS program, \u201cThinking Ahead\u201d will be used as a basis for this class. Bioethics Team meetings and proper use of the POLST will also be discussed.<\/p>\n

This is a 3-hour training in person at the SARC San Jose Office.<\/p>\n

Certificate for 3 Continuing Education Units will be available upon completion of the class and passing the post-test with 80% correct.<\/p>\n

Space is limited- 20 people maximum at this time.<\/strong><\/p>\n

2 people per facility to allow for more agencies to attend. <\/strong><\/p>\n
